PRESCHOOL ADMISSION REQUIREMENTS

For New Applicants
1. Duly accomplished Application Form with recent 2”x2” picture (studio taken with white background and name tag).
2. Age Requirement for:
       • Nursery: Must be 3 years old by October 31, 2024
       • Preparatory: Must be 4 years old by October 31, 2024
       • Kinder: Must be 5 years old by October 31, 2024
3. Certificate of Enrollment in Kinder (for Grade 1 applicants).
4. Photocopy of Birth Certificate (for Kinder and Grade 1).
5. Admission Fee: Php550.00

For Transfer Applicants (Grade 2-6)
1. Duly accomplished Application Form with recent 2”x2” picture (studio taken with white background and name tag)
2. For Grade 2 to Grade 6 applicants: Certified True Copy of Form 138 with Grades until the Third Grading Period
3. Admission Fee: Php550.00

ADMISSIONPROCEDURE

For New Applicants
Step 1: Accomplish the Admission Form and supply the necessary information and attach the requirements.
Step 2: Pay the Admission Fee at the Cashier’s Office, St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.
Step 3: Submit the accomplished Application Form and Official Receipt to the Preschool and Elementary (PSE) Guidance Counselor at the Office of the Guidance and Testing (OGT), St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.

For Transfer Applicants (Grade 2-6)
Step 1: Proceed to the Preschool and Elementary Principal’s Office, St. Martin Bldg. for the Form 138 evaluation.
Step 2: Accomplish the Application Form and attach the requirements.
Step 3: Pay the Admission Fee at the Cashier’s Office, St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.
Step 4: Submit the accomplished Application Form and Official Receipt to the Preschool and Elementary (PSE) Guidance Counselor at the Office of the Guidance and Testing (OGT), St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.
