JUNIOR HIGH SCHOOL ADMISSION REQUIREMENTS

For Grade 7 Applicants
1. Duly accomplished Application Form with recent 2”x2” picture (studio taken with white background and name tag).
2. Grade 6 Certificate of Enrollment (affixed with school seal)
3. Admission Fee: Php600.00

ADMISSION PROCEDURE

For Grade 7 Applicants
Step 1: Accomplish the Admission Form and attach the requirements.
Step 2: Pay the Admission Fee at the Cashier’s Office, St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.
Step 3: Submit the accomplished Application Form and Official Receipt to the Junior High School (JHS) Guidance Counselor at the Office of the Guidance and Testing (OGT), St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.

For Transfer Applicants
Step 1: Proceed to the Junior High School Principal’s Office, St. Albert Bldg. for the Form 138 evaluation.
Step 2: Accomplish the Application Form and attach the requirements.
Step 3: Pay the Admission Fee at the Cashier’s Office, St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.
Step 4: Submit the accomplished Application Form and Official Receipt to the Junior High School (JHS) Guidance Counselor at the Office of the Guidance and Testing (OGT), St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.
