PRESCHOOL ADMISSION REQUIREMENTS

For New Applicants
1. Duly accomplished Application Form with recent 2”x 2” picture (Studio taken with white background and name tag)
2. Age Requirement for:
• Nursery: Must be 3 years old by October 31, 2025
• Preparatory: Must be 4 years old by October 31, 2025
• Kinder: Must be 5 years old by October 31, 2025
3. Certificate of Enrollment in Kinder or Enrolment Registration Form (For Grade 1 applicant.)
4. Photocopy of Birth Certificate (for Nursery, Preparatory, Kinder and Grade 1 applicant)
5. Official Receipt of Admission Fee:
• Preschool: Php 550.00
• Elementary: Php 600.00

Note: The School may consider learners entering Kindergarten who will turn five years old from November 1 to December 31 on the condition that the learner shall:

i. Have completed an Early Childhood Care and Development (ECDD) Program for one (1) one school year; or
ii. Undergo Philippine Early Childhood and Development (ECD) Checklist during the enrollment period until the first week of the school year

For Transfer Applicants (Grade 2-6)
1. Duly accomplished Application Form with recent 2”x 2” picture (Studio taken with white background and name tag)
2. For Grade 2 to Grade 6 applicants: Certified True Copy of Form 138 with Grades until the Second Grading Period
3. Official receipt of Admission Fee: Php 650.00
4. Certificate of Good Moral Character

 

ADMISSION PROCEDURE

For New Applicants
Step 1: Accomplish the Admission Form and supply the necessary information and attach the requirements.
Step 2: Pay the Admission Fee at the Cashier’s Office, St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.
Step 3: Submit the accomplished Application Form with complete attachments to the Office of Guidance and Testing (OGT), St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.

For Transfer Applicants (Grade 2-6)
Step 1: Proceed to the Preschool and Elementary Principal’s Office, St. Martin Bldg. for the evaluation of Form 138.
Step 2: Accomplish the Application Form and attach the requirements.
Step 3: Pay the Admission Fee at the Cashier’s Office, St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.
Step 4: Submit the accomplished Application Form with complete attachments at the Office of Guidance and Testing (OGT), St. Thomas Aquinas Bldg., UST-Legazpi, Rawis Campus.
